This portal allows new patients to fill out the required paperwork to be seen by Dr. Meredith. All information submitted to this portal is confidential and accessible only by Dr. Meredith and her staff. Using the patient portal requires an email address. Your email address is confidential and will not be sold, shared, or used for advertising or any other non-medical purpose. Filling out new patient forms at home will be simple and reduce your wait time in our office on your first visit. You will still need to bring your insurance cards and a picture ID to the office at the time of your visit.

If you would prefer to fill out the paperwork at home and bring it into the office the new patient forms can be found to the right of this paragraph. Printing your forms at home will not require an email address. Filling out forms at home will be simple and reduce your wait time in our office on your first visit. You will still need to bring your insurance cards and a picture ID to the office at the time of your visit.
The Length of our Medical Questionnaire seems long and possibly excessive. However, we assure you that the questionnaire does help Dr. Meredith provide you the best possible foot care. Also, Some of our questions are required by our electronic medical records company for meaningful use. We apologize if this causes any inconvenience. Print at home option requires that you be able to open PDF format. |
